I think all of that is right except that they only play each other once. I think the SL clubs get the extra home fixture.

It's only 4th v 5th that play off. The top 3 and bottom 3 in the middle 8 go their separate ways automatically.

Yes. It's just a straight 80 minute shoot out to see who goes into Super League and who goes into Championship for 2016.
So bottom 4 in SL and top 4 in Championship join together to form middle 8. All points return to 0.
Everyone plays each other once. After 7 matches, the top 3 win a place in SL for 2016 and the bottom 3 are in the Championship.
4th v 5th play each other to decide the final spot in both leagues.
